The purpose of this assignment is for you to identify an issue of concern for your role as an advanced practice nurse and to formulate a potential policy change to address that issue. There are many potential issues which can influence your practice setting or other issues which may negatively affect the patients with whom you work. All of the course reading will help you to identify a topic for this assignment. You can think about the issue as related to your health promotion project. The policy you consider may be in reaction to the health promotion issue or something larger that is still related to that issue. There are hundreds of possible issues, but here is a list of a few to consider:

After you read the Introduction to the Guide to Physical Therapist Practice, focus on the section related to the International Classification of Functioning, Disability, and Health (ICF).  The ICF allows one to use standard language and framework to classify a patient and describe how people function in their daily lives.  As your written assignment, please apply the ICF model to one of your patients as follows:

  1. Please include a BRIEF summary of the basic facts about the patient case, i.e., overview of patient’s history, review of systems, etc.
  2. Briefly describe the ICF Model.
  3. Apply the model to your patient, i.e., the first part of the ICF model describes function and disability utilizing body functions and body structures and the changes that happen within them.  You would therefore need to understand what that means before you can apply it to your patient.  (See the ICF-Towards a Common Language guide that we’ve provided in the Module 1 readings for more help).
  4. Make sure to include all components of the ICF model.  Additionally, you may use information from both the patient’s perspective as well as the physical therapist’s perspective.  Please include ALL relevant information concerning your patient’s presentation.
  5. You may choose to use a diagram, table or figure to assist in demonstrating your understanding of the model, but it is not required.
  6. Once you have applied the ICF model to the case, choose a diagnostic label (a medical diagnosis or a ‘physical therapy diagnosis’) and provide your rationale for choosing that label.  For example, list the ICD-10 codes that apply to your patient and explain what the codes stand for and why you selected them.

Since some of you may choose to use the Guide to Physical Therapist Practice-3.0, you can add the preferred practice pattern that you feel best exemplifies your patient.  Although the preferred practice patterns are no longer noted in the latest version of the Guide, they are available for educational use and noted in the Module I recommended readings list for your benefit.

Please use Microsoft Word; no more than 5 pages of text double-spaced in narrative not bulleted format. Please submit your assignment in the drop box as an attachment with your last name and paper title (ex. leslie disablement.doc). Reference as appropriate, citing sources using APA format, and please include a separate title and reference page.

Patient-centered care is a great way to improve the quality of care being received. Not every patient responds the same way to the same techniques. Taking that into consideration, being more aware of each patients feelings of the conditions they are in, can really help to guide the practice put into place thereafter. Experience can help with this as well, as it gives the nurse more ideas of different ways to handle one situation.

Quality Improvement is looked at by leadership for the purpose of making processes in health care more efficient and therefore, save money (McEwen, M., & Willis, 2019). It takes data and statistical measures to investigate where performance can be improved. Essentially, if something isn’t working, more than likely it’s taking resources that can be spent elsewhere or used in a better way.

In the emergency department when I think of quality improvement, what comes to mind is re-admittance rates or “frequent flyers”. There is an alert on the patient’s chart during triage that notifies nurses if the patient has been to or in the hospital in the last 30 days and for what purpose. We don’t want people to keep coming to the ED for the same reason over and over. That is an overuse of the ED and abuse of resources. To prevent overuse and abuse, many MD’s collaborate with case managers and social workers on behave of the patient to provide follow-up resources and even assistance with a Primary care Physician. Although these things are set in motion, many people miss their follow-appointments and come back to the ED.

Leaders must look at the numbers and see where there can be improvements made. Most often, these improvements need to be taught to the staff so they can implement changes. Leaders should be forthcoming with the goals and able to encourage change by the staff members.

Discussion: Identifying and Engaging Stakeholders for Strategic Planning

Stakeholders can facilitate the success, or failure, of a strategic plan.

Identifying internal and external stakeholders and building productive relationships should be an intentional and well-conceived part of the strategic planning process. As a nurse leader-manager involved in strategic planning, it is important that you consider who could be impacted by a proposed change and how the change may affect individuals’ roles, responsibilities, and relationships. It is also essential to think about which stakeholders may be able to offer expertise, capital, and/or sway to help exercise the strategic plan and generate buy-in.

In this Discussion, you identify stakeholders who can assist with carrying forward a strategic plan.

Your Instructor has assigned you to a small group for this Discussion. Craft your stakeholder description independently; through the collegial exchange that follows, you will offer each other suggestions for refinement. As in Weeks 2 and 3, use this small group Discussion to delve deeply into the concepts being presented as they are foundational to this course.

To prepare:

Reflect on a planned change effort you have experienced.

Consider the following questions:

  • How do you think stakeholder involvement may have impacted the outcome of this planned change?
  • Were the right stakeholders involved? Did the stakeholders help to carry out the plan as needed?
  • Should other stakeholders have been engaged? If so, how could relationships with additional stakeholders have been cultivated to better support the plan?

Conduct additional research as necessary to strengthen your understanding of the process of involving stakeholders in strategic planning and to deepen your thinking about the organization. For instance, you may research organizations with similarities to Mountain View, and examine information related to stakeholder involvement.

Consider the following questions:

  • Which internal and external stakeholders should be involved in strategic planning related to this case study?
  • What assets and/or perspectives would each of these stakeholders offer to the process?
  • What role would each potential stakeholder play in helping to move forward a strategic plan?
  • When and how should the stakeholders become involved?
  • How would you cultivate relationships with these stakeholders to produce the best possible results?
  • How would the appropriate selection of stakeholders and their involvement promote the likelihood of successful adoption and implementation of a strategic plan?

Post a description of internal and external stakeholders who should be involved in strategic planning at Mountain View Health Center. Propose strategies for cultivating relationships with these stakeholders. Explain how their involvement would promote the likelihood of success for the strategic plan.
