The appropriate use of cell phones and other electronic devices is extremely important in business settings (and everywhere else) today. Please note that it is very discourteous to monitor your phone, email and text messages, or to take calls in the middle of a conversation with others — unless it is an emergency situation — and that includes checking out groceries with your cashier.

Think about the messages being sent: “You are not important enough to receive my full attention.” “I don’t care about you or our relationship enough to give you my full attention.” “Somebody else, somewhere else, is more important at this moment than you are.” “I’m ignorant and don’t know enough to turn my cell phone off or to pay attention to business.” “I can’t focus.” “I’m rude.” “I don’t care about you or what you have to say.” “Your time is not important to me.” “Your attention can wait.” “Your message is not important enough to receive my full attention.” And the list goes on. Please remember to use good form in all situations — the perception of others is their reality.

Chapter 4 Reality Check Billing and Reimbursement

Management of Accounts Receivable is a complex process which allows providers and their practices to get accurately, ethically, and legally reimbursed for services they provide to their patients. During the checkout process, all of the co-payments, coinsurance, and deductibles are collected from patients at that time. This is a major responsibility of the front desk office clerk to collect co-payments or make other appointments.

In Renal Practice Associates, one of the front desk clerks, Silvia, knows the patients very well. In fact, she knows when they are having difficulty paying co-payments and medical-related expenses. Some patients are in life-threatening End Stage III Renal Failure and on dialysis. Sylvia is aware the practice she works for is a big, successful one, and her rationale is no one will care about missing one or two payments from a patient, or stating on the patient account the bill is paid.

When patients with acute or chronic conditions check out at Silvia’s station, she will often tell them they owe no balance, and that she is happy to reschedule them for their next appointment.

After a few weeks of observing this behavior, Lily, her co-worker, is concerned. Lily has only been in her position for six months, while Sylvia has worked in the practice for ten years. Lily is not sure what to do or how to approach this problem. It’s obviously raising a red flag for her, but she can’t possibly lose her job. She’s not sure what to do, and feels these might be her options:

  1. Not say anything and ignore this practice is going on.
  2. Ask Sylvia if there is a special policy and procedure that she may not be aware of that allows some critically ill patients not to pay for their visit. If so, Lily should ask to see the policy and procedure herself so she is aware of it in writing.

Case Scenario:

Ms. Glitter goes to the emergency department (“ED”)
complaining of severe stomach pain. After physical examination, a CT of the
abdomen shows an abdominal aortic aneurysm. 
This is a weakening of the wall of the aorta which causes it to stretch
and bulge.  The wall of the aorta weakens
as a result of the stretching and can create a situation where if the wall of
the aorta ruptures, death will ensue within a matter of minutes.  The ED physicians inform her that the only
way to fix the aneurysm is via a surgical approach and that the chances of
survival are about 50/50. They also inform her that in making a decision about
whether to undergo the surgery, time is of the essence because of the risk of
rupture and imminent death. 

The woman is an exotic dancer; she worries that the
surgery will leave a scar on her abdomen that will negatively affect her
work.  As such, she refuses surgical
treatment. Even after much pressuring from the physicians, she adamantly
refuses surgery. Feeling that the woman is not in her correct state of mind and
knowing that time is of the essence, the surgeons decide to perform the
procedure without consent. They anesthetize her and surgically repair the
aneurysm. She survives, and sues the hospital for millions of dollars.

Stacy is a high school counselor. A Vietnamese student named Hanh, is assigned to her because of academic difficulties. Stacy observes that Hanh is slow and deliberate in his conversational style, and she immediately assigns him to a remedial speech class. In the course of their conversations, Han discloses to Stacy that his father wants him to apply to college and major in pre-med. Hahn is not sure that he even wants to attend college. Stacy gives John a homework assignment, asking him to tell his father that he no longer wants to pursue college plans and wants to follow a direction that appeals to him.

  • Was the fact that John spoke slowly and deliberately and indication that he needed a remedial speech class? Can you offer other explanations for Hanh’s slow and deliberate speech?
  • If you were Hanh’s therapist, how would you deal with the conflict between Hanh’s goals and his father’s expectations?
  • Was Stacy culturally sensitive when asking Hahn to directly confront his father?What other Alternatives were available?

Commentary

When there is a cultural difference between the counselor and client, counselors must familiarize themselves with how the client approaches counseling and avoid imposing their world view on the client. Although we hope the counselor would explore with Hahn his choice of a major in college and the conflict with his father over his educational and career plans, it is inappropriate for the counselor to tell Hahn what to do. Suggesting that Hahn directly confront his father has serious implications; Hahn’s father may never speak to him again if Hahn is confrontational. Our assessment skills need to in compass the cultural context and the consequences a proposed intervention on the client’s life.
