To Prepare
By Day 1 of this week, you will be assigned to a specific case study for this Case Study Assignment. Please see the “Course Announcements” section of the classroom for your assignment from your Instructor.
Also, your Case Study Assignment should be in the Episodic/Focused SOAP Note format rather than the traditional narrative style format. I’d like to preface my post by saying that any act of abuse on the elderly is repugnant and deserves equal prevention and intervention. As someone who has personally had to make reports to charge nurses, managers, and Adult Protective Services, mistreatment of elderly patients is a serious problem for a vulnerable population. According to the National Council on Aging (NCOA):
Approximately 1 in 10 Americans aged 60+ have experienced some form of elder abuse. Some estimates range as high as 5 million elders who are abused each year. One study estimated that only 1 in 14 cases of abuse are reported to authorities (NCOA, 2019).
That being said, I’d have to say the most challenging abuse types to prevent or report are emotional and financial exploitation. Cultural factors, presence of cognitive illness, and susceptibility to these forms of abuse make interviewing difficult and can often be a grey area for reporting.
There are many ways to advocate and protect these patients from abuse, but being observant to cues like sudden financial changes, social isolation, depression, anxiety, and changes in psyche around certain family members or caregivers. Money management programs, helplines, local emergency shelter resources, and multidisciplinary teams that include social workers, APS, civil legal services, etc. can help to coordinate these older adults into a safe, healthy situation.

Patients today are much sicker than they used to be. The advanced practice nurse needs to manage all comorbidities in the plan of care. In this post, I will discuss the patient’s health care needs and a recommended treatment plan, including pharmacotherapeutic choices. Also, I will present an education strategy to assist the patient in managing their disease conditions. The patient has a history of hypertension, myocardial infarction, hyperlipidemia angina, and diabetes type two. Her needs center around the management of these comorbidities. Also, I noticed that her serum creatinine is on the high end of the range if not outside of it. This fact is not surprising considering her diabetes. The case reports that she was doing well until about a month ago. It was tempting to consider adding another medication to the regimen. However, I believe that either the metoprolol is not high enough to manage her current disease state or she has been missing doses, and I am leaning toward the latter. If withdrawn suddenly, the drug can increase the incidence and intensity of anginal attacks. Metoprolol is the first-line drug for stable angina and should be working (Rosenthal & Burchum, 2018).
A review of her current drug therapy appears appropriate. Aspirin, simvastatin, and metformin are competent choices, and the doses are adequate. My plan would include an assessment of how she takes medication at home and manages them before making any changes. If this were in order, I would increase the metoprolol to 75 mg twice a day and set a follow-up appointment in two weeks. I prefer to increase the dose of metoprolol first before adding any new drug because of polypharmacy considerations, and the drug does not require renal adjustment (Epocrates, 2019). This fact is beneficial to consider because she has diabetes. Assuming that the patient has missed doses over the last month, the priority is to determine why. Costa et al. (2015) recommend explaining how to take medication, discussing reluctance to take drugs, and a conversation about the patient’s beliefs and knowledge about their health and treatment. I believe that this is a great strategy to use with the patient. Medication adherence is troubling and particularly so with the elderly who have multiple medications and conditions. I think it is essential for the patient and provider to be partners in the plan of care. The patient must agree to it and fully understand their conditions and the purpose of each medication.

Ethical Dilemma 3: An office worker has had a record of frequent absence. He has used all his vacation and sick-leave days, and has frequently requested additional leave without pay. His supervisor and co-workers have expressed great frustration because his absenteeism has caused bottlenecks in paperwork, created low morale in the office, and required others to do his work in addition to their own. However, the individual believes he is entitled to take his earned time and additional time off without pay. Is he right?
Ethical Dilemma 4: Rhonda enjoys socializing with fellow employees at work, but their discussions usually consist of gossiping about other people, including several of her friends. At first, Rhonda feels uncomfortable talking in this way about people she is close to; but then she decides it does no real harm, and she feels no remorse for joining in.
In conjunction with the readings, and within your teams, decide which ethical dilemma you believe is most problematic and why.
