Complete the Fluid disturbance table and answer the following ABGs (Arterial Blood Gases).
Assignment : Case Study Assignment: Assessing Neurological.
Imagine not being able to form new memories. This is the reality patients with anterograde amnesia face. Although this form of amnesia is rare, it can result from severe brain trauma. Anterograde amnesia demonstrates just how impactful brain disorders can be to a patient’s quality of living. Accurately assessing neurological symptoms is a complex process that involves the analysis of many factors.
In this Case Study Assignment, you will consider case studies that describe abnormal findings in patients seen in a clinical setting.
By Day 1 of this week, you will be assigned to a specific case study for this Case Study Assignment. Please see the “Course Announcements” section of the classroom for your assignment from your Instructor.
Also, your Case Study Assignment should be in the Episodic/Focused SOAP Note format rather than the traditional narrative style format. Refer to Chapter 2 of the Sullivan text and the Episodic/Focused SOAP Template in the Week 5 Learning Resources for guidance. Remember that all Episodic/Focused SOAP notes have specific data included in every patient case.
With regard to the case study you were assigned:
Review this week’s Learning Resources, and consider the insights they provide about the case study.
Consider what history would be necessary to collect from the patient in the case study you were assigned.
Consider what physical exams and diagnostic tests would be appropriate to gather more information about the patient’s condition. How would the results be used to make a diagnosis?
Identify at least five possible conditions that may be considered in a differential diagnosis for the patient.
Use the Episodic/Focused SOAP Template and create an episodic/focused note about the patient in the case study to which you were assigned using the episodic/focused note template.
Provide evidence from the literature to support diagnostic tests that would be appropriate for each case.
List five different possible conditions for the patient’s differential diagnosis, and justify why you selected each.
Neurological Cases for Week 9
Case 1: Drooping of the face: A 33-year-old female comes to your clinic alarmed about sudden “drooping” on the right side of the face that began this morning. She complains of excessive tearing and drooling on her right side as well.

There are many types of elder abuse. Choose two types and discuss challenges you perceive in abolishing them. Justify why you consider them as the most crucial. Discuss how you can advocate and protect the elderly from abuse.
I’d like to preface my post by saying that any act of abuse on the elderly is repugnant and deserves equal prevention and intervention. As someone who has personally had to make reports to charge nurses, managers, and Adult Protective Services, mistreatment of elderly patients is a serious problem for a vulnerable population. According to the National Council on Aging (NCOA):
Approximately 1 in 10 Americans aged 60+ have experienced some form of elder abuse. Some estimates range as high as 5 million elders who are abused each year. One study estimated that only 1 in 14 cases of abuse are reported to authorities (NCOA, 2019).
That being said, I’d have to say the most challenging abuse types to prevent or report are emotional and financial exploitation. Cultural factors, presence of cognitive illness, and susceptibility to these forms of abuse make interviewing difficult and can often be a grey area for reporting.
There are many ways to advocate and protect these patients from abuse, but being observant to cues like sudden financial changes, social isolation, depression, anxiety, and changes in psyche around certain family members or caregivers. Money management programs, helplines, local emergency shelter resources, and multidisciplinary teams that include social workers, APS, civil legal services, etc. can help to coordinate these older adults into a safe, healthy situation.
Why topic was chosen?

Post 1 article synopsis; defining key words and how article relates to chosen area of interest.
A citation and reference from a professional source are required for all discussion boards, for module two through module seven. Failure to include a reference for the initial post requires a 60 point deduction per the rubric, so please remember to meet all criteria.
For your Discussion Post, DO NOT FORGET TO INCLUDE AN ARTICLE SYNOPSIS defining keywords, in addition to answering discussion questions, when instructed. Review the rubric. To avoid losing points for Discussions, consider using headings to be sure you don’t miss anything (not required, just a suggestion)
Assignment 1: Hernandez Family Assessment
Assessment is as essential to family therapy as it is to individual therapy. Although families often present with one person identified as the “problem,” the assessment process will help you better understand family roles and determine whether the identified problem client is in fact the root of the family’s issues. As you examine the Hernandez Family: Sessions 1-6 videos in this week’s Learning Resources, consider how you might assess and treat the client family.
Assess client families presenting for psychotherapy
Review this week’s Learning Resources and reflect on the insights they provide on family assessment.
View the Hernandez Family: Sessions 1-6 videos, and consider how you might assess the family in the case study.
Note: For guidance on writing a comprehensive client assessment, refer to pages 137–142 of Wheeler (2014) in this week’s Learning Resources.
Address in a comprehensive client assessment of the Hernandez family the following:
History or present illness
Past psychiatric history
Substance use history
Family psychiatric history
History of abuse and/or trauma
Review of systems
Mental status exam
Note: Any item you are unable to address from the video should be marked “needs to be added to” as you would in an actual comprehensive client assessment
APA format. Scholar authors only. Please read all instructions.PowerPoint presentation with an infographic (graphic that visually represents information, data, or knowledge. Infographics are intended to present information quickly and clearly.) to educate others on the role of nurse as knowledge worker. I have include how to make the inforgraphic in the reference section. Also, I have attach my discussion question post that should be use in the Powerpoint.
The PowerPoint should Include the hypothetical scenario you originally shared in the Discussion Forum. Include your examination of the data that you could use, how the data might be accessed/collected, and what knowledge might be derived from that data. Be sure to incorporate feedback received from your colleagues’ responses.
To Prepare: Review the concepts of informatics as presented in the Resources. Reflect on the role of a nurse leader as a knowledge worker. Consider how knowledge may be informed by data that is collected/accessed.
The Assignment: Explain the concept of a knowledge worker. Define and explain nursing informatics and highlight the role of a nurse leader as a knowledge worker. Develop a simple infographic to help explain these concepts.

Your recommendations should apply the principles in Exhibit 6.5 on page 149 and Exhibit 6.6 on page 150.
Read a selection of your colleagues’ responses and respond to at least two of your colleagues on two different days who were assigned a different patient case study, and provide recommendations for alternative drug treatments to address the patient’s pathophysiology. Be specific and provide examples.
Women’s and Men’s Health, Infectious Disease, and Hematologic Disorders
Patients today are much sicker than they used to be. The advanced practice nurse needs to manage all comorbidities in the plan of care. In this post, I will discuss the patient’s health care needs and a recommended treatment plan, including pharmacotherapeutic choices. Also, I will present an education strategy to assist the patient in managing their disease conditions. The patient has a history of hypertension, myocardial infarction, hyperlipidemia angina, and diabetes type two. Her needs center around the management of these comorbidities. Also, I noticed that her serum creatinine is on the high end of the range if not outside of it. This fact is not surprising considering her diabetes. The case reports that she was doing well until about a month ago. It was tempting to consider adding another medication to the regimen. However, I believe that either the metoprolol is not high enough to manage her current disease state or she has been missing doses, and I am leaning toward the latter. If withdrawn suddenly, the drug can increase the incidence and intensity of anginal attacks. Metoprolol is the first-line drug for stable angina and should be working (Rosenthal & Burchum, 2018).
A review of her current drug therapy appears appropriate. Aspirin, simvastatin, and metformin are competent choices, and the doses are adequate. My plan would include an assessment of how she takes medication at home and manages them before making any changes. If this were in order, I would increase the metoprolol to 75 mg twice a day and set a follow-up appointment in two weeks. I prefer to increase the dose of metoprolol first before adding any new drug because of polypharmacy considerations, and the drug does not require renal adjustment (Epocrates, 2019). This fact is beneficial to consider because she has diabetes. Assuming that the patient has missed doses over the last month, the priority is to determine why. Costa et al. (2015) recommend explaining how to take medication, discussing reluctance to take drugs, and a conversation about the patient’s beliefs and knowledge about their health and treatment. I believe that this is a great strategy to use with the patient. Medication adherence is troubling and particularly so with the elderly who have multiple medications and conditions. I think it is essential for the patient and provider to be partners in the plan of care. The patient must agree to it and fully understand their conditions and the purpose of each medication.

Collaborate with your team, using Cisco Spark, email, phone meetings, or any collaboration tool you find useful or prefer. In your collaboration, consider the ethical dilemmas below and select 1 in which to conduct a deep drill.
Ethical Dilemma 1: A newspaper columnist signs a contract with a newspaper chain. Several months later, she is offered a position with another newspaper chain, offering a higher salary. Because she would prefer making more money, she notifies the first chain that she is breaking her contract. The courts will decide the legality of her action, but what of the morality? Did the columnist behave ethically?
Ethical Dilemma 2: An airline pilot receives his regular medical checkup. The doctor discovers that he has developed a heart murmur. The pilot only has a month to go before he is eligible for retirement. The doctor knows this and wonders whether, under these unusual circumstances, she is justified in withholding information from the company regarding the pilot’s condition.
Ethical Dilemma 3: An office worker has had a record of frequent absence. He has used all his vacation and sick-leave days, and has frequently requested additional leave without pay. His supervisor and co-workers have expressed great frustration because his absenteeism has caused bottlenecks in paperwork, created low morale in the office, and required others to do his work in addition to their own. However, the individual believes he is entitled to take his earned time and additional time off without pay. Is he right?
Ethical Dilemma 4: Rhonda enjoys socializing with fellow employees at work, but their discussions usually consist of gossiping about other people, including several of her friends. At first, Rhonda feels uncomfortable talking in this way about people she is close to; but then she decides it does no real harm, and she feels no remorse for joining in.
In conjunction with the readings, and within your teams, decide which ethical dilemma you believe is most problematic and why. In your teams, discuss the ideas of “good vs. evil,” “wrong vs. right,” and “ought/should be vs. what is.” Form the readings, discuss the ways in which Augustine and Aquinas would have solved the problem based on lecture and course reading material. In what ways do Augustine and Aquinas differ and why?
You may wish to meet throughout the week to share ideas. Create a report of your findings as individuals and as a team. The report should be approximately 2 pages accompanied by a 2-minute oral presentation, using VoiceThread or a PowerPoint narrated slide show. Rubric
