The patient does not normally use oxygen at home. Patient has a past medical history of emphysema, COPD, DM II, anxiety, HTN, hyperlipidemia, and cigarette smoking 1 1/2 PPD x 50 years. Patients states that keeping the head of the bed up has helped his breathing but has now caused lower back pain. Patient has been taking ibuprofen at home for this but it has not helped. Therefore, the physician has ordered hydrocodone/acetaminophen (Norco) 2 tabs PO every 4-6 hours as needed for pain. Mr. Johnson is now on 2liters of oxygen via nasal cannula. He can receive respiratory albuterol nebulization treatments every 4-6 hours as needed. He can ambulate with assist X1 due to unsteady gait a frequent stops to catch his breath.    You find Mr. Johnson in his room hunched over the overbed table watching TV. He states this position is more comfortable for his breathing but he’s fine. Upon auscultation of posterior chest, his lung sounds are clear but diminished bilaterally. He has a continuous pulse oxygenation probe on this finger to monitor for hypoxia. His oxygen saturation is between 90-92% at rest 2L via nasal cannula. You note a cap refill of 4 seconds and slight clubbing of the fingers.    After breakfast Mr. Johnson, complains of lower back pain and requests the Norco tablets. You obtain his vital signs, which are 99.2 F, 90 Bpm, respirations 26, BP 156/82, and O2 saturation is 92% with 2L via nasal cannula. The Norco is given and within an hour the patient states he feels better and is ready to ambulate in the hall.
